In June 2024, the U.S. Senate Finance Committee released a sweeping report detailing abuse at residential treatment facilities operated by major healthcare companies, including Universal Health Services and Sequel, which operate or operated several such facilities in Alabama. The report revealed systemic sexual, physical, and emotional abuse, unsafe conditions, inhumane confinement practices, and neglect throughout these facilities.
Examples of alleged abuse, and of conditions likely to lead to abuse, include the following:
Alabama Clinical Schools
- In January 2023, a lawsuit alleged that an 11-year-old boy was physically abused by employees at least four times.
An employee allegedly hit him in the head, face, and neck.
Another employee allegedly kicked and stomped him in the stomach and face.
The employees allegedly threatened to kill him and retaliated against him for reporting the abuse to the Alabama Department of Human Resources (DHR). DHR conducted an investigation and confirmed that the abuse allegations were true.
Hill Crest Behavioral Services
- In 2017, surveillance videos and accounts from nearly 50 former patients, foster children, family members, and employees exposed instances where staff tackled, dragged, choked, and unnecessarily sedated patients.
The facility faced serious allegations, including inadequate employee training and unsanitary conditions. One incident allegedly involved a teenager attempting self-harm due to a lack of supervision and understaffing. Staff also alleged that they faced pressure to falsify records, downplay incidents, and misrepresent staffing levels.
Laurel Oaks Behavioral Health Center
- In 2022, a lawsuit alleged a young resident was sexually assaulted at Laurel Oaks.
The resident’s father claimed that Laurel Oaks did not take his son to the hospital after the assault. The father took his son to a nearby hospital himself.
The lawsuit alleged that Laurel Oaks mishandled the incident, claiming the victim’s attacker had a history of sexual violence.
In January 2026, a settlement was reached with the victim’s family.
- In 2014, a 17-year-old patient was charged with felonies for sexually assaulting another patient.
In 2011, a 40-year-old employee was convicted of sexually assaulting a teenage patient.
Sequel Owens Cross Roads
- In June 2022, a lawsuit alleged that a 16-year-old girl was abused at Owens Cross Roads.
The girl allegedly called 911 and the Alabama Disabilities Advocacy Program.
In response, staff allegedly pinned her to the ground, placed their knees on her back, and instructed another resident to hold her arms down.
While restrained, other residents allegedly kicked her in the face and head while other staff stood by without intervening.
The girl sustained a fractured jaw and bruising to her face and ribs.
- Another victim was allegedly isolated for five consecutive days.
- One girl allegedly attempted to hang herself with a telephone cord.
Three Springs
- In 2020, a report uncovered filthy and hazardous living conditions at the facility, including feces on the floor and blood-stained walls.
It also detailed lack of proper security, inadequate supervision and medical care, and illegal isolation and restraint.
- In 2017, a 28-year-old youth care worker was sentenced for sexual contact with a 13-year-old boy.
